We’re looking for a Nurse Practitioner or Physician Assistant/Associate to join our dynamic, multidisciplinary team at our outpatient diabetes clinic — where evidence-based medicine meets genuine, patient-centered care. As a key member of our team, you’ll:

• Lead meaningful patient visits and craft individualized care plans targeting glycemic control, disease prevention, and long-term wellness
• Partner with endocrinologists, diabetes educators, dietitians, and nursing staff to deliver truly holistic care
• Empower patients and their families with the knowledge and confidence to manage diabetes through education on lifestyle, medications, and self-care
• Order and interpret labs, fine-tune treatment regimens, and track outcomes that matter
• Help shape the future of our program by contributing to quality improvement initiatives and clinic growth

About you:

• Nurse Practitioner or Physician Assistant/Associate with ability to obtain/maintain New Hampshire licensure

• National certification as a PA, FNP, or AGNP

• Clinical experience in endocrinology, diabetes care, primary care, or related specialty

• Strong clinical judgment, communication, and patient education skills
